Verification Forms
Complete these forms only if you have been notified by our office that you have been selected for verification. You can review your file at mybama.ua.edu to see if you are required to complete any of these forms. You may submit the requested forms/documentation to our office by using the Student Financial Aid Verification Document Upload portal, by mail, by fax, or in person. See 2026-2027 Verification for more information on how to submit.

Consortium Agreement
A consortium agreement is a contract between the student, the host institution and The University of Alabama. This agreement allows The University of Alabama to process Federal Student Aid for the student while taking courses at another university.
The consortium agreement form is available from the Office of Student Financial Aid and Scholarships. The student should complete the student section and forward the consortium agreement to his/her Academic Advisor for approval and return to Student Financial Aid and Scholarships for processing. Student Financial Aid and Scholarships will process and administer all financial aid for the student in accordance with its policies and procedures, and the host institution agrees not to provide financial aid to the student registered under this agreement. Financial aid will be disbursed to the student. The student will receive any unused portions as a refund.

Parent PLUS Loans
Initial Request:
Complete your PLUS Loan application at https://studentaid.gov. We must receive the results of your completed 2026-2027 FAFSA (also at https://studentaid.gov) before we can process your PLUS Loan request.
The 2026-2027 Parent PLUS Loan application is available now.
The Summer 2027 Parent PLUS Loan application will be available March 29, 2027.
Changes to a Parent PLUS Loan:
Rather than complete a new application to request an update to your Parent PLUS Loan, submit your request in writing with our dedicated form.
Request to Change Parent PLUS Loan
Graduate PLUS Loans
Initial Request:
Complete your Grad PLUS Loan application at https://studentaid.gov. We must receive the results of your completed 2026-2027 FAFSA (also at https://studentaid.gov) before we can process your PLUS Loan request.
The 2026-2027 Grad PLUS Loan application is available now.
The Summer 2027 Grad PLUS Loan application will be available March 29, 2027.

Private Loans and Other Aid
If you will not apply for federal aid but need our office to process another type of aid for you, inform us in writing to avoid an extensive delay.
Special Circumstances/Cost of Attendance Increase Requests
If there are new or changed circumstances not correctly reflected on your Free Application for Federal Student Aid (FAFSA) or your Cost of Attendance, review the forms below to see if your circumstance can be considered.
