Verification Forms

Complete these forms only if you have been notified by our office that you have been selected for verification. You can review your file at mybama.ua.edu to see if you are required to complete any of these forms. You may submit the requested forms/documentation to our office by using the Student Financial Aid Verification Document Upload portal, by mail, by fax, or in person. See 2025-2026 Verification  for more information on how to submit.

Consortium Agreement

A consortium agreement is a contract between the student, the host institution, and The University of Alabama. This agreement allows The University of Alabama to process Federal Student Aid for the student while taking courses at another university.

The consortium agreement form is available from the Office of Student Financial Aid & Scholarships. The student should complete the student section and forward the consortium agreement to his/her Academic Advisor for approval and return to Student Financial Aid & Scholarships for processing. Student Financial Aid & Scholarships will process and administer all financial aid for the student in accordance with its policies and procedures, and the host institution agrees not to provide financial aid to the student registered under this agreement. Financial aid will be disbursed to the student. The student will receive any unused portions as a refund.
