Verification Forms
Complete these forms only if you have been notified by our office that you have been selected for verification. You can review your file at mybama.ua.edu to see if you are required to complete any of these forms. You may submit the requested forms/documentation to our office by using the Student Financial Aid Verification Document Upload portal, by mail, by fax, or in person. See 2024-2025 Verification for more information on how to submit.
Consortium Agreement
A consortium agreement is a contract between the student, the host institution, and The University of Alabama. This agreement allows The University of Alabama to process Federal Student Aid for the student while taking courses at another university.
The consortium agreement form is available from the Office of Student Financial Aid. The student should complete the student section and forward the consortium agreement to his/her Academic Advisor for approval and return to Student Financial Aid for processing. Student Financial Aid will process and administer all financial aid for the student in accordance with its policies and procedures, and the host institution agrees not to provide financial aid to the student registered under this agreement. Financial aid will be disbursed to the student. The student will receive any unused portions as a refund.
PLUS Loans
Federal Parent Loan for Undergraduate Students (PLUS) Applications:
We must receive the results of your completed FAFSA before we can process your federal Direct PLUS Application. Read this important information for 2024-25 Parent PLUS processing before you apply for PLUS loans.
Apply for a Parent PLUS loan for Fall 2024 and/or Spring 2025.
The Summer 2025 PLUS Loan application will be open March 25, 2025.
Changes to a Parent PLUS Loan:
- Securely submit a request to update your current federal Direct PLUS loan request with the Online Request to Change Parent PLUS form with DocuSign.
- If you prefer, print the Paper Request to Change Parent PLUS Loan form, complete and submit it to Student Financial Aid to change your loan request.
Federal Graduate PLUS Loan Applications:
We must receive the results of your completed FAFSA before we can process your Federal PLUS Application. Read this important information for 2024-25 Grad PLUS processing before you apply for PLUS loans.
Apply for a Graduate PLUS loan for Fall 2024 and/or Spring 2025.

Private Loans and Other Aid
If you will not apply for federal aid but need our office to process another type of aid for you, securely complete the Waiver of Federal Aid form with DocuSign to avoid an extensive delay. If you prefer, print the Paper Waiver of Federal Aid form, complete and submit it to Student Financial Aid.
Special Circumstances/Cost of Attendance Increase Requests
If there are new or changed circumstances not correctly reflected on your Free Application for Federal Student Aid (FAFSA) or your Cost of Attendance, review the forms below to see if your circumstance can be considered.
